Patient Resources

New patients are encouraged to print out a copy of our OWC New Patient Information Form and bring a completed copy to your first appointment. Please arrive 15 minutes prior to your scheduled appointment time to complete registration procedures. We ask that new patients bring your medical insurance card and photo ID to your appointment. At each visit, you will need to provide your current insurance card(s) to ensure correct insurance filing on your behalf. Any applicable co-pay as payment is expected at the time of service. We encourage all patients to be familiar with their insurance policies and recommend calling your insurance company prior to your appointment if you have questions regarding your plan. We participate with most insurance companies. Please call our office for more information. If you need to cancel or reschedule your appointment, please contact us as soon as possible at 913-764-6262.
